Do you have Blackberry Desktop manager installed? You need that installed in order to upgrade your BB OS. Once it's installed, install the OS 4.5 (leave your BB disconnected) and when it asks if you want to start desktop manager, don't start it. Then move the vendor.xml from C:\Program Files\Common Files\Research In Motion\AppLoader to somewhere else (I just moved it to my desktop).
From there, connect your BB, and run the desktop manager. It should see that you've got updates. (The 4.5 OS comes with "Documents 2 Go" which is nice). You can go through what's gonna be installed, and add/remove items from there. Pretty much leave it as is.
Once you click next, the update will start. It'll take about 20-30 minutes for the entire process to complete (unless your PC is sloooooooooooow).
Once the upgrade is complete, your phone will "re-activate", but other than that, it should work like it did before, only newer OS.
